The VMR provides you with your own copy of all the information relevant to your pregnancy including your test results. This record is yours to write on and should be kept with you and taken to whichever care givers you see during your pregnancy. Regardless of who is providing your care ask them for your own VMR so that you can retain responsibility for your own information.
